Photograph and Video Release Form

I do hereby give permission to North Texas Community Foundation to photograph or videotape me/use a photograph or video supplied by me (or a photo or video of a minor, for whom I am legally responsible).

I give this permission with the understanding that these photographs or video may be used in any of the Community Foundation’s presentations, print pieces, advertisements, electronic communications, website, or social media spaces for informational and promotional purposes. 

The photographs or video may be viewed by a wide variety of audiences, which includes, but is not limited to Community Foundation staff, volunteers, donors, professional advisors, charitable organizations, news media, the general public, etc.
